File a Report

Unless you provide your name or the name of the alleged perpetrator, this information is used for the purpose of statistical reporting.

If you have been subjected to a physical, emotional, sexual or gender-based harassment or misconduct, EOU strongly encourages you to report your experience to the University and/or local law enforcement. EOU’s Title IX Coordinator is trained to work with individuals who report physical, emotional, sexual or gender-based misconduct, and provide information about resources, support service and procedural options.


We believe it is the right of the victim/survivor of interpersonal violence to decide if they wish to report an incident, and how much information they wish to share about the incident.

You can make a report at any time. However, you are encouraged to make a report as soon as possible after an incident. Retaliation against any person for making a report or participating in an investigation is strictly prohibited. If you decide not to make a report, or if you are unsure about reporting, you are still entitled to confidential resources provided by the University. You can also request changes to your academic, housing, transportation and working situations.

  • Submit a Title IX Report Form.
  • File a report in person with the Title IX Coordinator, Inlow Hall 113a, 541-962-3476 or titleix@eou.edu.
  • Make an anonymous report through a confidential employee. Not sure who are confidential employees? Visit the Student Info for Reporting page or connect with our Privileged Campus Advocate.

You are able to file a report with the police as well as the university. You can file a criminal complaint with local law enforcement. Our Privileged Campus Advocate, Title IX Coordinator, and any other Campus advocates are available to assist you.

Statement on retaliation

EOU prohibits retaliation against individuals who engage in reporting a complaint of sexual harassment, sexual misconduct, dating or domestic violence, and/or stalking. Retaliatory behavior is regarded as additional misconduct and may lead to increased sanctions.
