Aug. 30, 2021 – Important updates for students: Testing requirements and deadline for COVID immunization verification

**IMPORTANT UPDATES** 

The FDA approved the Pfizer-BioNTech COVID-19 Vaccine today (Monday, August 23), which means EOU’s vaccination requirement is now in effect. To meet the requirement, all students who plan to work, learn or engage with EOU on-campus in La Grande this fall need to be fully vaccinated or have an approved exemption on file by October 22. The requirement does not apply to fully online or remote students.

Vaccination timeline:

You are considered to be fully vaccinated two weeks after your final dose.

  • Moderna: 1st dose by September 10; 2nd dose by October 8
  • Pfizer: 1st dose by September 17; 2nd dose by October 8
  • Janssen: only dose by October 8

COVID-19 Immunization Verification Form due Sept. 10

All students who will be taking in-person classes, living in the residence halls, attending on-campus events, or using on-campus resources must fill out and return the COVID-19 Immunization Verification Form by 5pm, September 10

The form gives you the following options:

  • Vaccination verification: If you have already received a COVID vaccine, please complete the COVID-19 Immunization Verification Form and make a copy or take a photo of your vaccination card or other documentation. Send both the form and the image of your documentation to the Student Health Center (shc@eou.edu).
  • In the process of getting vaccinated: Please include the dates of your dose(s) (can be in the future). You will need to provide a copy of your vaccination card or other documentation once you have received your final shot.   
  • Medical exemption: If you need a medical exemption, please complete the medical exemption section of the COVID-19 Immunization Verification Form and get a note from your primary care provider. Send both the form and the doctor’s note to the Student Health Center (shc@eou.edu).
  • Non-medical exemption: If you choose not to get a COVID vaccine for non-medical reasons, please complete the exemption section of the COVID-19 Immunization Verification Form and submit it to the Student Health Center (shc@eou.edu). 

Mandatory testing for unvaccinated students

Students who are unvaccinated must provide a negative COVID test before they move into the residence halls, start in person classes, attend on campus events, or use on campus resources.   

Prior to the term, unvaccinated students must do the following:

  • Take a PCR COVID test no earlier than five days before engaging on campus. Examples include moving into the halls, Week of Welcome, and starting classes (if living off-campus). 
  • Send test results to saffairs@eou.edu.

Note: Tests older than five days will not be accepted. We encourage you to consider the length of time it will take to get your results back. Ask your provider for the timeline and plan accordingly.

Exceptions: 

  • Fully vaccinated students
  • Unvaccinated students who have had a positive COVID test within the past 90 days. Please send your test results to saffairs@eou.edu
