PLEASE NOTE:

Payment in advance is required for official transcripts. The cost is $5 for each transcript ordered.

There is no charge for unofficial transcripts.

Credit Card Payment may be made by going to http://www.eou.edu/staccts/

(using student ID#) or by calling Student Accounts at 541-962-3590.

Education records are protected by the Family Education Rights and Privacy Act (FERPA). We must have your signature to release a transcript for any reason, even to you.

We process requests in the order they are received. Certain times during the term have more requests than others.
If you have any questions you may contact us at 541-962-3504.

If you are unable to download this form, you can submit a request, again either by mail or fax by sending us the following information.

• Student ID number or Social Security Number
• Full name and any other names used
• Address
• How many transcripts you need
• Where you want your transcripts sent
• Your signature (required to release transcripts)

• Transcripts will not be printed before payment is received.
• There is a $5 fee for each transcript ordered.  There is no fee for unofficial transcripts.
• Transcripts will not be printed if there are holds on the students account.  Holds must be cleared by the appropriate office.
• Official transcripts can not be faxed.
• Your signature is required for release of transcripts.
