Have you been contacted about submitting an Immunization History?

EOU is required to obtain immunization records from all of our admitted students.  Please help us to comply with this Oregon Administrative Rule by making sure your vaccination history regarding MMR (Measles, Mumps, Rubella) is up to date and on file at the Student Health Center.  Since immunization records are considered part of a medical record – the Student Health Center is now designated as the collection point for these files.

It is simple to comply with this records request:

1)  Visit the Student Health Center webpage regarding immunization form and facts: https://www.eou.edu/health/immunizations/. When you fill in the form, please be sure that your name and student ID# is legible and be sure to sign the form!

2)  Attach a copy of your vaccination record (signed by a health care practitioner) to your immunization form and deliver both forms to the Student Health Center on the northwest corner of the EOU Campus.

If your vaccination history is incomplete, stop by our Student Health Center and speak with one of the health care practitioners about options (testing for immunity, getting vaccinated, etc.).

Not an on campus student?

If you feel that you should be exempt from this requirement because you are not taking campus courses – please contact saffairs@eou.edu so we can review why your name/account were part of the download!  Unless the coding is corrected – you will continue to receive similar messages each term requesting you submit immunization history!  Please provide your name and student ID# in your message.

We need to have this resolved before the end of the term!
