After completing the AIM Application, you will receive an automated email that encourages you to make an appointment with Taylor Smith, Assistant Director of Disability Services, to discuss your needs and the accommodations for which you qualify.\u00a0 Intake appointments can be held in person, on Zoom, or over the phone. Appointments can be scheduled by contacting the Accommodations Specialist at 541-962-3235 or disabsvc@eou.edu.<\/p>\n\n\n\n<h3 class=\"wp-block-heading has-text-align-center\"><strong>Common Accommodations:<\/strong><\/h3>\n\n\n\n<h5 class=\"wp-block-heading has-text-align-center\"><em>Please note: The accommodation(s) a student receives are dependent upon their documented disability<\/em>.<\/h5>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><h2 id=\"at-1500\" class=\"c-accordion__title js-accordion-controller\" role=\"button\">Alternative Formats (E-Text\/ Audio)<\/h2><div id=\"ac-1500\" class=\"c-accordion__content\">\n<p>The Disability Services Office provides alternative text formats for students who have been approved for this accommodation.&nbsp; Audio Text is delivered through a text-to-speech program called Kurzweil 3000. Kurzweil 3000 reads digital files aloud and provides other useful tools such as bookmarking, highlighting, and note-taking. If a student qualifies to receive audio text (E-Text), our office will email the student a username and password to access Kurzweil 3000, along with instructions on how to upload files to the website. If the student has any issues navigating the website, they are encouraged to call the Accommodations Specialist at 541-962-3235.<\/p>\n\n\n\n<p>Please Note: If a student qualifies to receive Alternative Text Accommodation, they must sign an Alternative Text Agreement Form at the beginning of every term. The Alternative Text Agreement Form will be sent to the student&#8217;s email. &nbsp;<a href=\"https:\/\/docs.google.com\/a\/eou.edu\/forms\/d\/e\/1FAIpQLSd26-xAWJiTX-0uFRAaC6X7M-tTfpxKoHGhYXuKv6YiN-EHwA\/viewform\">Alternative Text Agreement Form EXAMPLE<\/a><\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><h2 id=\"at-1501\" class=\"c-accordion__title js-accordion-controller\" role=\"button\">Assistive Technology<\/h2><div id=\"ac-1501\" class=\"c-accordion__content\">\n<p>Our office also provides a speech-to-text program called Dragon Naturally Speaking. All computers located in the Disability Services Office are equipped with this program. All EOU students also have access to the voice typing tool located in Google Docs.<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><h2 id=\"at-1502\" class=\"c-accordion__title js-accordion-controller\" role=\"button\">Alternative Testing<\/h2><div id=\"ac-1502\" class=\"c-accordion__content\">\n<p>Alternative Testing Arrangements are made on an individual, case-by-case basis during the initial intake appointment with the Assistant Director of Disability Services. A student may receive one or many of the following testing accommodations:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Time and a half 1.5X<\/li>\n\n\n\n<li>Double Time 2.0X<\/li>\n\n\n\n<li>Minimum Distraction<\/li>\n\n\n\n<li>Computer<\/li>\n\n\n\n<li>Calculator as permitted by instructor<\/li>\n\n\n\n<li>Reader<\/li>\n\n\n\n<li>Scribe<\/li>\n<\/ul>\n\n\n\n<p>Alternative Testing Policy<\/p>\n\n\n\n<p><strong>For tests and quizzes administered In-Person (on-campus):<\/strong><\/p>\n\n\n\n<p>The student should schedule to take their test in the Testing Center by emailing their office at Testing@eou.edu. Students should email their office at least two days prior to the exam\/quiz date.<\/p>\n\n\n\n<p>In the email the student should include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Student\u2019s Name<\/li>\n\n\n\n<li>Student\u2019s School ID<\/li>\n\n\n\n<li>Course Name and Number<\/li>\n\n\n\n<li>Instructor<\/li>\n\n\n\n<li>Specify other accommodations they qualify for and would like to utilize during testing (i.e. reader or a scribe)<\/li>\n\n\n\n<li>Date and Time the exam is given in class<\/li>\n<\/ul>\n\n\n\n<p><strong>(Please Note: The student must schedule to take the exam at the same time it is given in class. The student is only able to change the testing time and date if it has been approved by the instructor.)<\/strong><\/p>\n\n\n\n<p>The Testing Center will email the instructor and request a copy of the exam. On the testing date, the student should go to the Testing Center Office instead of class to take the exam\/quiz.<\/p>\n\n\n\n<p><strong>For tests and quizzes administered through Canvas (Could apply to Online, Remote, Hybrid, and In-Person Courses):<\/strong><\/p>\n\n\n\n<p>Once your professor receives the academic accommodation notification through their email, they are asked to extend the time allotted on all upcoming quizzes and exams. Students are asked to check that all tests and quiz times have been extended through Canvas. If you notice your test and quiz times have not been extended, please contact the Accommodation Specialist at 541-962-3235.<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><h2 id=\"at-1503\" class=\"c-accordion__title js-accordion-controller\" role=\"button\">Note Taking Support<\/h2><div id=\"ac-1503\" class=\"c-accordion__content\">\n<p>Students who qualify for this accommodation will be provided notes through a personalized Google Drive Folder. The Google Folder will include the student\u2019s name and term. (i.e. Samuel Smith- Spring 2020 Notes). On-campus students can also receive physical copies of notes upon request. If a student is interested in becoming a Note Taker for the Disability Services Office, they should apply through the <a href=\"https:\/\/www.eou.edu\/career\/\">Career Services<\/a> website.<\/p>\n<\/div><\/div>\n\n\n\n<div class=\"wp-block-pb-accordion-item c-accordion__item js-accordion-item no-js\" data-initially-open=\"false\" data-click-to-close=\"true\" data-auto-close=\"true\" data-scroll=\"false\" data-scroll-offset=\"0\"><h2 id=\"at-1504\" class=\"c-accordion__title js-accordion-controller\" role=\"button\">Tutors\/Study Partners\/ Mentors<\/h2><div id=\"ac-1504\" class=\"c-accordion__content\">\n<p><strong>TUTOR:<\/strong> The Disability Services Office (DSO) works collaboratively with the Learning Center and utilizes many of the same resources. If the student needs a tutor for one of their classes, the DSO will show them how to schedule a tutoring appointment through <a href=\"https:\/\/eou.mywconline.com\/\">WC online<\/a>. If the student is having difficulty scheduling an appointment or accessing a tutor, they should let the DSO know immediately. Please contact the DSO at 541-962-3235 or disabsvc@eou.edu.<\/p>\n\n\n\n<p><strong>STUDY PARTNER:<\/strong> The DSO will provide the student with a Study Partner. The DSO will email the student with their Study Partner&#8217;s name and contact information. It is the student\u2019s responsibility to email or phone the Study Partner in order to set up meeting times throughout the term. If the student is having difficulties getting in contact with their Study Partner, they should let the DSO know immediately (541-962-3235\/ disabsvc@eou.edu).<\/p>\n\n\n\n<p><strong>MENTOR:<\/strong> The DSO will provide the student with a Mentor. The DSO will email the student with their Mentor\u2019s name and contact information. It is the student\u2019s responsibility to email or phone the Mentor in order to set up meeting times throughout the term.
