July 19, 2021 – COVID-19 vaccination verification & exemption survey for employees

As EOU continues to plan for resumption of normal operations in Fall 2021, while ensuring the continued health and safety of our university community, the Human Resources Department is gathering information about proof of COVID-19 vaccination of employees working on EOU’s main campus. This is the first step in verifying proof of vaccination or requesting exemption.

Beginning 60 days after any COVID-19 vaccine receives full FDA approval, all EOU employees assigned to work on the main campus in La Grande will be required to either:

(a) provide proof of being fully vaccinated against COVID-19, or

(b) request and obtain an approved medical or nonmedical vaccination exemption.

This process is consistent with existing practices that allow students to request medical or non-medical exemptions. EOU is applying this same methodology to employees regarding COVID-19 vaccination.

To facilitate our return to normal operations this Fall, the first step is for all employees to complete the Vaccination or Vaccination Exemption Survey the next time they log into Mountie Hub or by August 11. The survey describes the proof of vaccination confirmation process and the procedure for requesting an exemption from the mandatory vaccination requirement that will soon be put in place.

Visit eou.edu/coronavirus for more information about EOU’s vaccination requirement and ongoing pandemic response. If you have questions about completing the survey, contact Human Resources at hr-group@eou.edu
