Before your Perkins Loan will be released to your account you
must complete the Perkins Master
Promissory Note, the Personal
and Confidential Information Sheet, and the Perkins
on-line Entrance Counseling
Please follow these steps:
Step 1:
Access the Perkins Loan Master Promissory Note here.
Complete items 1 - 5, print all 4 pages, sign and date page 1.
Step 2:
Access the Personal Information Sheet here.
Complete all items, print, and sign.
Step3:
Access the Perkins Entrance Counseling here.
Follow the instructions for completing the counseling. Write your confirmation
number next to your signature on the Personal Information sheet.
Staple all 5 pages together and submit either by mail to the
address on the Promissory Note, or in person at the Student Accounts
Department. Original signatures
are required. Please do not FAX.
