• Identify the mission or central purpose of the unit (1 page):

• Describe the unit in sufficient detail(1-2 pages):

• Identify all significant changes that have occurred in the unit since 1998 (1-2 pages).

• Analyze and appraise the results of any assessments of the unit since 1998.  How were these assessments used to make the unit more effective? (IV – VII should be 8-10 pages)

• As a result of those significant changes motivated by assessment, what are the current strengths of the unit?

• What assessments are in progress or are planned for the unit, and why?

• What commendations / recommendations for action does the unit make to itself?
