Name Changes
Luckily, no form is needed if you get married and want your married name to appear on your transcripts. Simply send an email to registrar@eou.edu and we will change your name. The same is true if you wish to change back to your maiden name.
Any name change other than for marriage requires a copy of the "General Judgment of Change of Name and Order to Post" filed with the Circuit Court.
PLEASE NOTE: Contact the following offices to notify them of your name change. Each office uses a different database, and not all of them talk to each other.
