Graduation Application
Please read these instructions carefully before you fill out your graduation application. Try to file three terms in advance of your intended graduation date, but no later than the second Friday of the term prior to which you plan to graduate (i.e., finish your degree requirements). Complete the graduation application form with your advisor. Each line below correlates with a line you must fill out on the graduation application.
Name & Student ID# - Print your name exactly as you wish it to appear on your diploma & write your student ID number on the line indicated. If you don't know your student ID number, you can use your social security number.
Address, City, State, Zip - This is the address where your graduation letter will be sent.
Phone Number & Email - We will use these to contact you if it is necessary.
Intended Graduation Date - Write the year in the space after the term in which you will complete your degree requirements. Do not use June just because you want to participate in the commencement program. Remember, commencement and graduation are two different things.
Degree - If you are receiving a Bachelor in Music, you will check the BMUS box; if you are receiving a Bachelor of Arts degree, you will check the BA box; and if you are receiving a Bachelor of Science degree, you will check the BS box. Then write in the name of your major. [Please note: If your major is Liberal Studies, you may be required to declare minors. Check with your advisor.]
Concentration if applicable - Some majors have concentrations, which will be noted on your transcript. Please check the catalog to see if your major has a concentration. Write the concentration on this line. [Please note: This is not the same as a minor. If you are pursuing a minor, you will need to complete and submit a minor recognition form for it to be noted on your transcript.]
Hometown/state... - Your name, degree, major and hometown will be listed in the commencement program. If you wish your hometown to be listed, please write it on this line.
Degree program requirements... - You can graduate under the catalog in which you matriculated, or a subsequent catalog. Please write in the year of the catalog for your degree requirements.
Required courses to complete - Use these lines to write in the course prefix, number, title, and credits of the courses that you need to complete, including any major requirements, general education requirements, upper division requirements, and electives. Be sure to include the courses that you are enrolled in but have not yet received a final grade. Please note that the Registrar's office or the Dean's office may include courses which are deemed necessary for you to complete before your degree will be awarded.
Now your advisor will need to sign this form. Then it will need to come to the Registrar's Office.
