Course by Arrangement
This form is also known as a "Reading and Conference" form and an "Individualized Study" form.
There are some courses which must be arranged through the faculty instructor. Examples of these courses might be certain Research courses, Reading & Conference courses, Capstone courses, or Special Topics courses.
As the student fills out this form, leave the line number space blank. A line number (or course reference number) will be created by the Registrar's office. The faculty instructor must sign the form, and then the Dean of the College must sign. Then the student will bring the form to the Registrar's office to add the course.
