Registration Form
For On-Line and On-Site Courses
This form should be used if you are trying to register for courses after the term has started, and you have not completed the non-admit information online, or you have not completed your admissions and you wish to take 8 credits or less for this term. You will also need to submit the instructor's approval for you to register in the course. An email from the instructor to our on-line registration coordinator is acceptable. The email can be sent to registrar@eou.edu.
