Graduation Application
Complete and file your graduation application three terms in advance of your intended graduation date, but no later than the second Friday of the term prior to which you plan to graduate (i.e., finish your degree requirements). Complete the graduation application form with your advisor.
Print your name exactly as you wish it to appear on your diploma.
Write the address where your graduation letter and diploma will be sent.
Some majors have concentrations. List your concentration as it will be noted on your transcript. Please note: This is not the same as a minor. If you are pursuing a minor, you will need to complete and submit a minor recognition form for it to be noted on your transcript.
Your name, degree, major and hometown will be listed in the commencement program. If you wish your hometown to be listed, please write it on the Hometown/State line.
You can graduate under the catalog in which you matriculated, or a subsequent catalog. Please write in the year of the catalog for your degree requirements.
List the required courses to complete including the course prefix, number, title, and credits of the courses that you need to complete, including any major requirements, general education requirements, upper division requirements, and electives. Be sure to include the courses that you are enrolled in but have not yet received a final grade. Please note that the Registrar's office or the Dean's office may include courses which are deemed necessary for you to complete before your degree will be awarded.
