OTHER FORMSThis page contains forms which you may need AFTER you have been awarded funds. If you need the application forms, please choose "How to Apply" from the sidebar on the left. If you have used this page previously, you should choose RELOAD when you use a PDF file for the new copy to load on your screen. The forms on this page are provided in PDF format. This page will allow you to choose specific forms. If you want forms in Corel WordPerfect vs. 8 or Microsoft Word 2000, you can go to the bottom of this page to download the entire packets as self-extracting ZIP files.
FORM for STUDENTS and/or Emergency Medical Service organizations:The main form you will need is the Travel Reimbursement Request form. It is used for all classes except conferences (when travel is not reimbursed). Please use your mouse to click on the form below. Please note, there is a $200 limit per person per class. We will therefore need to know who drove and when they drove as compared to others in a carpool situation, to make sure all people are appropriately reimbursed. FORMS for EDUCATION SERVICE PROVIDERS:Use these forms if you provide education. You might be a community college, an AHEC or an Emergency Medical Service organization of some kind which is providing your own continuing education or First Responder education. These forms will be sent to you when you are informed that funding has been awarded for the class you are offering. IF you need extra copies, you can download them below.
Students sign the rosters once the class has gone beyond the "drop" date at a community college or once the class is complete for a continuing education or First Responder course. FORMS in MICROSOFT WORD 2000
