If you have received an acceptance e-mail, congratulations!  

Check-in begins in the main lobby of Badgley Hall at 8:30 a.m. on Saturday, October 23, 2010. At that time, you will receive your investigator packets and meet your team. The opening session is held at 9:00 a.m. and the fun begins!

Below are the consent forms that must be signed before you can participate in Girls in Science. Please read each section carefully. All Girls in Science students and their parent(s) or guardian(s) must sign and or initial the document, front and back, in the appropriate places:

2. Photograph/Background Information Release Permission

3. Participation Release

Mail, scan and e-mail to hchandler@eou.edu or fax this form to Northeast Oregon Area Health Education Center by Friday, October 15th, 2010.  If we do not receive the above completed documents by Wednesday, October 20th at the latest, you will not be eligible to attend Girls in Science.
