Complete appropriate Admission Form.  Admission forms should be submitted when 60 or more credit hours have been completed (Click here for all forms listed below).

  • Two EOU Minors
  • One EOU minor and One Minor from another accredited institution
  • Individualized Program
  • Business and Health Promotion Preapproved Program
  • Environmental Studies Preapproved Program
  • Early Childhood Education Preapproved Program
  • Organized Psychology Preapproved Program

Complete second page entitled, “Degree Plan Template.”

List all courses in both minors/subject fields.

If unsure of when a course may be scheduled, list the course, but leave the term/year/grade empty putting TBA. (Click here to see a sample of a completed Liberal Studies Admission Form).

Reminders:

  • Program must include at least 60 credit hours of which 36 must be upper division (300 or 400 level credit).
  • All EOU minors require a minimum of 10 EOU credits with in each minor.
  • All EOU minors require a minimum of 20 EOU credits, of which 10 must be upper division in the major.
  • SIGN your admission form with your name.



Off-Campus Students:

Scan and e-mail, mail, or deliver both forms to your academic adviser who signs it and then e-mails or sends your forms to the EOU campus for final approval by the Liberal Studies Director.

On-Campus Students:

Obtain signature of your discipline representative for each minor. Take admission form to your academic adviser for signature. Turn in completed forms to the Liberal Studies Director.
