To request a new list for your class or other campus related purpose, please complete this form and click the submit button.

Please submit a completed form for each list you are requesting.

Students

Have a faculty advisor complete this form - a list cannot be created solely at a student's request.

Note that the list name must include a hyphen (-) so as not to duplicate an existing account name or mailing alias.

Subscription Policy: (OPEN is the easiest)	OPEN: All requests to subscribe or unsubscribe are automatically processed CLOSED: List owner (you) approves new subscriptions, but not unsubscribe
Moderation option (LIST is usually best)	LIST: only people on the list can post without your approval YES: list owner approves every single message before it goes to the list NO: anyone in the world can post anytime to the list - not recommended
