· informing students/families about MCD/OHP (distributing flyers…
or even creating flyers to be distributed; having an OHP info night)

· talking to students/families about health or prevention services/programs
that are covered by MCD (ie, physical, mental, dental health care)

· helping families to complete OHP application; explaining eligibility;
informing students/families of where they can go to sign up for MCD or
MCD-covered services

· assisting in the identification of children who could benefit from health
services that are covered by MCD (ie, screening)

· referring students/families for health services

· monitoring or evaluating medical care as part of IEP or 504 plan

· making follow-up contacts to ensure that a child received a medical or
mental health service

· coordinating the delivery of physical or mental health care

· working/collaborating with other agencies to identify gaps, reduce
duplication, coordinate services pertaining to MCD-covered services (ie,
planning for or conducting the dental screening, coordinating immunizations,
FRCs meeting with agencies to discuss outreach, FRCs planning the outreach
days as long as health-related services are involved)
