Original contracts are kept on file by the Accountant 1in Inlow Hall, Room 208.

Forms

Personal Services Agreement Short Form for $5,000 and under AND services requiring ONLY one payment.
Personal Services Agreement Long Form for payments over $5,000 (which require bids) or for ANY contract requiring more than one payment.
W-9 Forms must be submitted by all Personal Contract Service employees of the university.
Amendments to the Personal Services Agreement are required if an additional service is being performed by a contractor who already has a contract for the fiscal year OR if the total amount being paid is higher than the original contract.
