Academic Intervention Instructions

A hold has been placed on your account.  In order to make adjustments to your class schedules or conduct other University business for the term, you must do the following:

On Campus Students

1. Fill out a College Success Assessment form. 
   Assessment Form.
2. Contact your academic advisor for an appointment where both of you can review your plans for addressing performance issues and discuss steps to take towards academic success.
3. After the appointment, the hold will be removed.
4. In two or three weeks check back in with your advisor.

Online / Offcampus Students

1. Fill out a College Success Assessment form.  Click here for the form. 
2. Contact your Advisor or Center Director to arrange a phone appointment where both of you can review your plans for addressing performace issues and discuss steps to take towards academic success. 
3. After the phone appointment, your advisor will remove your hold.
