Faculty Procedure

1.  Contact your advisees who are on Academic Probation (a list will be provided to you each term) to set up an advising appointment.

 

2.  Review College Success Assessment questionnaire that the student has completed (you will recieve this through email or the student will bring to the appointment).  Click here to view the form.  Please focus on the student success plan portion of the form.

3.  Utilize success assessment tools

4.  Call the advising office 962-3520 or the registrars office 962-3504 to have the hold removed.

5.  Follow up with student via phone or email in four weeks.
