Administrative Faculty – Forms

 

Position Description - Each year, in conjunction with the annual review report, all Administrative faculty members need to review and if necessary update their position description. This position description asks for a general description of the position, a description of the specific duties with percentages of time spent on each duty, number of employees supervised and a few other housekeeping items. If an employee’s position description is current this form does not need to be completed.

 

Performance Apprasial -  The annual Performance Evaluation form must to be completed annually. The form asks for an evaluation of goals and objectives set for the current year; goals and objectives to be set for the next year; a summary of management responsibilities, leadership and service to campus; and leadership and service to the community for the year. This is the primary document, along with the supervisor’s recommendation, used when step advancement consideration is requested.

 

Grievance Procedure - The purpose of this grievance procedure is to provide for open communication, which promotes the equitable resolution of employment disputes. This procedure will be construed to protect the substantive rights of interested persons to meet appropriate due process standards and to assure that EOU complies with all applicable laws and regulations.

 

Sabbatical Request  - Administrative Faculty are eligible to request sabbatical leave in the same manner as teaching faculty.  These leaves may be requested by any employee who has six years of continuous service at the University. Below are links to the sabbatical leave policy, frequently asked question page and the sabbatical request.  Sabbatical frequently asked questions (FAQ’s)
